What it is

TB-500 is a synthetic version of the active portion of a naturally occurring protein called Thymosin Beta-4, which is found in virtually every cell in your body and plays a key role in how cells organize, move, and respond to injury. When tissue is damaged, Thymosin Beta-4 is released at high concentrations at the injury site, where it helps coordinate healing by activating repair cells, modulating inflammation, and supporting new tissue formation. TB-500 is specifically the fragment corresponding to amino acids 17 through 23 — the region researchers identified as the most biologically active. An important distinction: this is not the same as Thymosin Alpha-1 (peptide 03 in this guide), which is a completely different compound that works through immune modulation. TB-500 is also on the WADA prohibited list, meaning anyone subject to competitive sports drug testing cannot use it. The fragment itself has not been studied in human clinical trials, though the full Thymosin Beta-4 molecule has been in Phase 2 cardiac research with modest, mixed results.

Animal Studies

Human Studies

The full TB4 molecule has Phase 2 cardiac trial data and an FDA Orphan Drug Designation. The TB-500 fragment specifically has NOT been studied in human clinical trials. Human evidence for TB-500 is extrapolated from the full molecule, which has a different molecular profile and different pharmacokinetics.

Long-Term Safety

Doesn't exist for the TB-500 fragment. The WADA ban reflects both its performance-enhancing potential and the absence of an established safety record. The cancer-related cell migration concern is biologically grounded: Thymosin Beta-4 is known to be overexpressed in several human cancers, which is why cancer history is an essential screening question.
