Food sources, meaningful testing, and when tablets or injections make sense.

Vitamin B12 supports blood cells, nerves, and DNA synthesis.2 Low intake and poor absorption are different routes to deficiency.3 Your plan should distinguish dietary coverage from treatment.

Low intake and poor absorption need different plans

A vegan eating pattern needs dependable fortified foods or supplements.1 Absorption disorders can require treatment despite an animal-food-containing diet.3

Start by recording your usual sources and every product containing B12. Then separate prevention from treatment: a multivitamin chosen to cover intake is not automatically an adequate plan for a diagnosed deficiency.

A large label number is not the absorbed amount

The adult RDA is 2.4 mcg daily, rising to 2.6 mcg during pregnancy and 2.8 mcg during lactation. These are nutritional reference intakes for healthy people, not deficiency-treatment prescriptions.2

Only a small fraction of a high-dose oral supplement is absorbed. A 1,000 mcg tablet is therefore not equivalent to delivering 1,000 mcg into the bloodstream.1 Dose comparisons should include route, schedule, cause of deficiency, and follow-up.

Form names do not establish a winner

Cyanocobalamin, methylcobalamin, adenosylcobalamin, and hydroxocobalamin are B12 forms. ODS identifies no absorption advantage across forms and no efficacy advantage for sublingual over swallowed preparations.1

An injection can be appropriate when the cause or severity calls for it. That decision is separate from paying more for a product described as active, methylated, or premium.

A common supplement form, converted by the body.1

Methylcobalamin / adenosylcobalamin

Active forms without established superior supplement absorption.1

Hydroxocobalamin

A form used in prescribed replacement.1

Distinguish treatment, delivery, and enhancement

The evidence questions below address different populations and outcomes. A rise in a blood concentration is useful information, but it is not by itself proof of more energy or better memory.

Does treating deficiency matter?

The cause and symptoms guide treatment

NICE recommends replacement for confirmed or sufficiently suspected deficiency, with the route informed by cause. Autoimmune gastritis and some gastrointestinal surgeries can require long-term intramuscular treatment. Follow-up includes symptoms and whether the underlying cause persists.3

A plan should answer both how to replace B12 and how to avoid recurrence.

Can oral treatment work as well as injections?

Blood-level evidence supports an oral option in selected patients

A 2018 Cochrane review included three trials totaling 153 participants over three to four months. High-dose oral replacement produced similar or higher B12 concentrations than injections. Certainty was low, and the trials did not report clinical symptom or quality-of-life outcomes.4

These findings support discussing oral treatment, not assuming every cause of deficiency can be managed with an over-the-counter tablet. Severe symptoms and the reason for malabsorption still matter.3

Will a higher B12 level improve how I feel?

Do not equate biochemical correction with enhancement

A trial randomized 201 adults aged 75 or older with moderate biochemical deficiency but no anemia or neurological or cognitive symptoms to 1 mg oral B12 daily or placebo for a year. B12 concentrations rose, but measured neurological and cognitive outcomes did not improve.5

The result does not argue against treating symptomatic deficiency. It does show why a memory or energy promise cannot be inferred from a higher laboratory value. Fatigue has many possible explanations; use an assessment rather than treating the marketing term “energy” as a diagnosis.

A normal blood count does not rule out deficiency

NICE advises considering symptoms and risk factors together and specifically warns against excluding B12 deficiency just because anemia or enlarged red cells are absent. Ideally, diagnostic blood samples are collected before replacement begins; disclose supplements already taken.3

Borderline results may prompt methylmalonic acid testing; kidney impairment can independently raise MMA.1 Also investigate why deficiency developed.

Do not let a supplement trial delay care

New numbness, balance changes, or worsening neurological symptoms merit prompt assessment. NICE says not to delay replacement while awaiting results when megaloblastic anemia and neurological symptoms are suspected.3

Metformin can lower B12, particularly with longer exposure or higher doses. The UK medicines regulator advises testing when deficiency is suspected and considering periodic monitoring in people at risk. Continue prescribed metformin while discussing assessment and correction with your clinician.6

No tolerable upper intake level has been established for B12.2 That reflects available safety evidence, not demonstrated benefit from escalating a dose.

If your goal is dietary coverage, identify a reliable source and an achievable routine. If you have symptoms or an abnormal result, bring the actual test report, medicine list, and supplement doses to your appointment. For an existing treatment plan, agree on what symptom change and follow-up findings would count as improvement.

Questions people ask

Can I be deficient without anemia?

Yes. NICE specifically says that absence of anemia or macrocytosis does not exclude B12 deficiency.3

Is methylcobalamin always better?

No absorption advantage is established across supplemental forms.1

Does a 1,000 mcg tablet mean 1,000 mcg is absorbed?

No. Only a small fraction of a large oral dose is absorbed.1

Should I stop metformin if my B12 is low?

Discuss treatment of the deficiency with your clinician; regulator advice supports continuing metformin when otherwise appropriate.6

Can I replace prescribed injections myself?

Discuss the reason for injections and a monitored alternative first. Oral-versus-injection trials provide limited evidence about symptom outcomes.4
