One nutrient family, several forms
Folate is the family name. Folic acid is a form used in fortification and supplements; 5-MTHF is another supplemental form.1 These names belong in one comparison, but evidence for a clinical outcome should remain attached to the form actually studied.
For pregnancy planning, the critical question is whether the product supplies the recommended folic acid, not simply whether the front label says prenatal or methylated.34

Leafy greens, legumes, and asparagus contribute natural folate. Enriched grains and fortified products can add folic acid.1

Why 400 mcg and 667 mcg can describe the same product
Dietary folate equivalents account for different absorption. One mcg of food folate equals one mcg DFE; 0.6 mcg folic acid consumed with food also equals one mcg DFE. Thus 400 mcg folic acid taken with food corresponds to about 667 mcg DFE.1
The adult RDA is 400 mcg DFE, pregnancy 600 mcg DFE, and lactation 500 mcg DFE. These nutritional requirements are distinct from the folic-acid-specific prevention recommendation.23
When comparing two labels, write down serving size, mcg DFE, actual mcg folic acid if present, and the ingredient form. Do not compare the largest printed numbers without their units.

Methylated does not mean universally better
CDC states that common MTHFR variants do not prevent people from processing folic acid. Folic acid is the form demonstrated to help prevent neural tube defects; common variants are not a reason to avoid it.4
This distinction does not mean 5-MTHF lacks nutritional activity. It means a claim about preventing a birth defect requires evidence for that outcome. A prescription folate regimen also should not be replaced with a supplement merely because the names sound similar.

Contributes to dietary folate equivalents through foods such as greens and legumes.1
Folic acid
The form used in the preventive recommendation for people who plan to or could become pregnant.3
5-MTHF / L-methylfolate
A supplemental folate form. CDC does not recommend avoiding folic acid because of common MTHFR variants.4

Three questions that should not be collapsed into one
Pregnancy prevention, a common genetic variant, and cardiovascular treatment are different contexts. The evidence below keeps the studied population and intervention visible.
What is the preventive recommendation?
Strong support for timely folic acid
The 2023 USPSTF recommendation advises 400–800 mcg folic acid daily for people planning to or who could become pregnant. The critical period starts at least one month before conception and extends through the first two to three months of pregnancy.3
A previous affected pregnancy or another very high-risk circumstance needs an individualized plan.3 Adequate folate nutrition remains relevant throughout pregnancy.2
Does an MTHFR variant mean folic acid will not work?
Common variants do not justify that conclusion
CDC reports that 400 mcg folic acid daily raises folate levels across common MTHFR genotypes. Its guidance explicitly rejects the idea that these variants require avoiding folic acid.4
Do not treat a common variant as proof of a need for high-dose methylfolate. Rare metabolic disorders are a different clinical question, requiring specialist interpretation rather than a generic supplement rule.
Does folate prevent cardiovascular disease?
A benefit in one setting is not a universal supplement indication
CSPPT randomized 20,702 adults with hypertension in China and no prior stroke or myocardial infarction to enalapril with 0.8 mg folic acid or enalapril alone. Over a median 4.5 years, first strokes occurred in 2.7% versus 3.4%, an absolute difference of 0.7 percentage points.6
The finding concerns a particular population and an addition to blood-pressure treatment. It does not establish that everyone should add folate for heart health, nor that lowering a homocysteine result guarantees better outcomes. Diet, baseline folate status, and background fortification affect applicability.

The upper limit is not a daily target
For adults, the general upper limit is 1,000 mcg daily of folic acid from supplements and fortified foods, excluding naturally occurring food folate. It is not 1,000 mcg DFE.2 Review high-dose or methylfolate regimens with a clinician rather than assuming a different form removes safety concerns.
Large supplemental folate intakes can correct the anemia of B12 deficiency without correcting its nerve damage, potentially obscuring the problem.2 Persistent fatigue or neurological symptoms deserve assessment of possible causes, including B12.
Folate can interact with methotrexate used for cancer. Antiseizure medicines and folate can affect one another, and sulfasalazine can impair folate absorption. Let the prescriber set the plan for these combinations.5

Read the complete prenatal or supplement label
Review overlapping products. Prior affected pregnancies or medicines that alter folate metabolism can require a different plan from the routine preconception recommendation.3
For everyday food planning, use a varied mix of the listed foods. For a diagnosed deficiency, establish why it occurred and what follow-up will demonstrate correction. A gene report alone does not describe your current nutrient status.
