This page explains how to exercise your privacy rights with Nuvitru Wellness LLC. Your rights are described in our Privacy Policy and, for health information, the Consumer Health Data Privacy Notice.
7.1 Rights You May Exercise
Depending on your state of residence, you may request that we:
- Confirm and access — tell you whether we process your personal information and give you a copy.
- Correct — fix inaccurate or incomplete personal information.
- Delete — delete personal information we hold about you, subject to legal and record-keeping exceptions (for example, clinical record-retention requirements).
- Port — provide your data in a portable, readily usable format.
- Opt out — stop targeted advertising, any "sale" of personal information (we do not sell personal information), or profiling with significant effects.
- Withdraw consent (consumer health data) — withdraw consent to our collection or sharing of consumer health data, and obtain a list of third parties with whom such data has been shared, where state law provides these rights.
7.2 How to Submit a Request
Submit a request by any of the following: email hello@nuvitru.com with the subject line "Privacy Request"; call (512) 501-2611; or mail Nuvitru Wellness LLC, Attn: Privacy, 3313 Ranch Road 620 S, Suite 200, Austin, TX 78738. Please state your name, contact information, your state of residence, and the specific right(s) you wish to exercise.
7.3 Verification
To protect your information, we must verify your identity before acting on a request — typically by matching information you provide against what we have on file, or by confirming through your client portal account. An authorized agent may submit a request on your behalf with proof of authorization; we may still verify directly with you.
7.4 Timing
We will respond within the time required by your state's law — generally 45 days, extendable once by an additional 45 days for complex requests (we will tell you if we need the extension). Opt-out and consent-withdrawal requests are honored as promptly as the law requires.
7.5 Appeals
If we decline your request, we will explain why. You may appeal by replying to our decision (or emailing hello@nuvitru.com with subject "Privacy Appeal") within a reasonable time. We will respond to appeals within the statutory period (generally 45–60 days). If your appeal is denied, you may contact your state Attorney General; we will provide a mechanism to do so in our appeal response.
7.6 No Discrimination
You will not be denied services, charged different prices, or provided a different quality of service because you exercised a privacy right.
