This consent explains virtual (telehealth) services provided by Nuvitru Wellness LLC. By booking or attending a virtual session, you acknowledge and agree to the following.
11.1 What Virtual Care at Nuvitru Is
Virtual sessions connect you with Nuvitru's practitioners — clinical nutritionists, dietitians, and collaborating licensed providers — by secure video, phone, and portal messaging. Nuvitru's virtual services are wellness and nutrition services, not emergency care, primary medical care, or (unless expressly arranged with a licensed physician or therapist and separately consented) medical or mental-health treatment.
11.2 Benefits and Risks
Benefits include convenience, access from anywhere in the U.S., and continuity of support between sessions. Risks include: technology failures that interrupt or delay care; the practitioner's inability to perform a physical exam, which may limit assessment; rare security incidents affecting electronic communications despite safeguards; and delays in response to portal messages (messaging is not monitored continuously and is never for emergencies).
11.3 Emergencies
Virtual care is not for emergencies. If you experience a medical or mental-health emergency, call 911 or go to the nearest emergency room. If a practitioner believes you are in danger during a session, they may contact emergency services and share the minimum information necessary to protect you.
11.4 Licensure and Location
Practitioners provide services consistent with the licensure and scope-of-practice laws of the state where you are located at the time of your session. You agree to tell us your physical location at the start of each session and to update us if you move. Some services may be unavailable in some states; we may need to modify, pause, or refer out your care if your location changes.
11.5 Your Responsibilities
You agree to: provide accurate, complete health information and keep it current; participate from a private, safe location (not while driving); maintain a physician for medical diagnosis and treatment, and inform them of your work with Nuvitru; and follow up as recommended, including on referrals for medical evaluation.
11.6 Technology and Privacy
Sessions are conducted through secure, encrypted platforms (including our Healthie client portal). We do not record sessions without your explicit consent, and you agree not to record without ours. Your information is handled per the Consumer Health Data Privacy Notice. Please use a private device and network; we cannot control the security of your own equipment.
11.7 Care Team Communication
You consent to communication among your Nuvitru care team (e.g., nutritionist, therapist, collaborating physician) so they can coordinate your plan. You may limit this by written request, understanding it may affect the coordinated-care model.
11.8 Minors
Virtual services for minors require the consent, and typically the attendance, of a parent or legal guardian.
11.9 Fees and Cancellation
Virtual sessions follow the same fee, rescheduling, and no-show rules as in-person appointments (§8.3).
11.10 Voluntary Consent
Virtual care is optional. You may decline or withdraw from virtual services at any time and request in-person alternatives (available at our Austin clinic) without penalty beyond standard scheduling policies. You may withdraw this consent in writing at any time, effective prospectively.
